What Hair Restoration Surgery Should I Choose?

When it comes to hair transplants, the most popular method today is called Follicular Unit Extraction (FUE). During an FUE procedure, individual hair follicles are excised from your scalp and implanted (one by one) into your desired area. Your existing hair is used to treat hair loss! How Much does a Hair Transplant Cost?

How Painful are Hair Transplants?

Hair transplants are pain free post local anaesthetic administration in both the donor and recipient areas on the day of surgery. Our patients report varying levels of pain/discomfort for this initial anaesthetic administration ranging from a scratch like feeling to a elastic band flick.

Post surgery day, the pain can last between 1-3 days however most of our patients report that paracetamol takes care of this pain. For some patients that paracetamol doesn’t fully kill the pain, we recommend to our patients to get over the counter co-codamol for something a bit stronger. Why Get a Hair Transplant if You're Transgender?

There are two key reasons why you may be interesting in getting a hair transplant: first, to reshape the hairline in a fashion more suitable to a person’s gender, and second, to halt signs of previous hair loss and pattern balding.

One of the biggest concerns facing the transgender community is gender dysphoria; the feeling that a person’s biological sex does not match gender identity. Hair transplant procedures can, potentially, reduce these feelings in patients.

Male to Female Transition

For many MTF patients, hair restoration surgery can be used to combat signs of balding that may have manifested pre-transition. One of the key hair procedures we can do is hairline lowering. This is where hair follicles are supplied into areas where hair has previously receded pre-transition.

The aim of this is to offer a more feminine appearance, in which hair new hair grows further down the head. This offers a permanent solution for those who have already experienced balding.

At Harley Street Hair Transplant Clinics, we also offer hair loss medication such as finasteride and minoxidil, which is designed to reduce androgen (DHT) level to slow down hair loss as well as encourage hair growth via a vasodilator like minoxidil. Female to Male Transition

For those undergoing FTM transition, we offer a number of solutions that tackle male pattern hair loss and the desire to achieve a more masculine look.

During the transition process, hormone therapy given to patients can cause symptoms of male pattern baldness. As increased testosterone is directly linked with male pattern hair loss, it is not surprising that FTM patients, post-transition, may experience signs of balding. This is caused by DHT (Dihydrotestosterone), which attaches itself to a hair follicles and causes hair to unalterably shed.

Male and Female Hairlines - The Different

Masculine and feminine hairlines differ in shape, position, and density. These differences are influenced by hormonal patterns, bone structure, and aesthetics, and are often considered in hair transplants or facial gender affirmation procedures. Understanding these distinctions is important for hairline restoration, scalp design, or gender-affirming surgery.

A masculine hairline typically features:

  • A higher frontal position on the forehead
  • A receding or M-shaped pattern, especially at the temples
  • Sharper angles and a more defined edge
  • Hair loss that progresses with age due to testosterone and DHT

Ftm surgery may involve modest recession of the corners – Sometimes paired with beard transplants to enhance masculinity – Testosterone therapy may alter natural growth patterns.

A feminine hairline is naturally:

  • Lower on the forehead
  • Rounded or oval-shaped, with soft curvature
  • Evenly dense across the frontal scalp
  • Less likely to recede, especially in early adulthood

For mtf, the hairline is lowered and rounded – Temple peaks are filled in to remove M-shape – Often combined with forehead contouring.

Hair Loss Medication for Transgender Patients

For trans women, the goal is to reduce androgenic hair loss and possibly regrow thinning hair. Common medications include:

  • Finasteride – It blocks DHT (the main hormone responsible for male pattern baldness). It works well alongside oestrogen to prevent any further hair miniaturisation.
  • Dutasteride – Stronger DHT blocker than finasteride. It blocks both type I and II 5-alpha-reductase enzymes.
  • Topical Minoxidil – Increases blood flow encouraging follicle growth – Works regardless of hormone status – Best for vertex and crown thinning.
  • Spironolactone – An anti-androgen that blocks testosterone at the receptor level. Commonly prescribed to MTF patients as part of HRT. May also help reduce hair shedding – Not used as a primary hair regrowth agent but supports overall hormone balance.
