Crown hair loss is among the most visible and psychologically significant patterns of male pattern baldness. A crown hair transplant relocates permanent, DHT-resistant follicles from your donor zone to the vertex, restoring natural density in an area that is technically demanding to treat well. A crown hair transplant uses the FUE (follicular unit extraction) technique to harvest individual follicular units from the back and sides of the scalp, where hair is genetically resistant to DHT, and implant them precisely into the thinning or bald area at the top of the head.

Unlike the hairline, where grafts are placed in a relatively simple plane, the crown requires the surgeon to replicate the natural vertex whorl. Every patient has a whorl point at the crown from which hair radiates outward in a circular pattern. Grafts in the centre of this whorl must be placed at a near-perpendicular angle, while follicles placed further from the centre shift progressively to a flatter angle and a changing rotational direction. Getting this wrong produces a result that looks transplanted regardless of density.

A second challenge unique to the crown is the density illusion. The crown sits on a curved surface, and photographs or mirrors compress the apparent size of the thinning zone. In clinic, the actual surface area is almost always larger than the patient expects. A moderate crown patch, roughly coin-sized in a mirror, typically requires 1,500 to 2,500 grafts to achieve adequate coverage. More advanced crown loss can require 3,000 grafts or more, often across two sessions.

Crown hair loss rarely stabilises early. In most patients it continues expanding year on year, which makes long-term planning as important as the surgical technique itself. Transplanting the crown without accounting for future loss risks creating an island of density surrounded by progressively thinner native hair, which looks unnatural and is difficult to correct.

For this reason, our surgeons assess your family history and the likely trajectory of your hair loss at consultation. In most cases where both the hairline and crown are thinning, we recommend addressing the hairline first. The hairline is more visible and frames your face; treating it first also preserves your donor supply for the crown later, once your loss pattern has stabilised. The exception is where crown loss is clearly more advanced than the hairline, or where the patient is in their 40s or 50s with a long-settled pattern.

For women with crown thinning, the assessment differs. Female crown loss tends to be diffuse rather than a defined patch, and donor density must be checked carefully before proceeding. The FUT technique is often preferred for women as it avoids shaving the donor area. Before your surgery, we hold an initial consultation with one of our surgeons. At this stage, your treatment plan, graft requirements, costs, and clinic location are all confirmed.

During the procedure itself, you will first be given a local anaesthetic so you will not feel anything. Hair follicles are then extracted from your donor area, typically the back of the scalp, and implanted individually into the crown. The number of grafts required depends on the extent of your crown loss: an early-stage patch may need 1,500 to 2,000 grafts, while more advanced loss at Norwood 4 or 5 may require 3,000 grafts or more.

After the procedure, you will receive a full aftercare package guiding you through your recovery. FUE leaves no linear scar and most patients feel comfortable returning to office work within 5 to 7 days. Strenuous activity should be avoided for 2 to 3 weeks. The transplanted hair will shed at 4 to 6 weeks, which is entirely normal, before new growth begins from around 3 months. Full results at the crown typically take 12 to 18 months to mature.

The cost of a crown hair transplant is determined by the number of grafts required to achieve adequate coverage. Because the crown surface area is consistently larger than it appears in a mirror or photograph, graft requirements, and therefore costs, are typically higher than patients initially anticipate. When compared to other treatment methods, there is nothing else available on the market that restores hair and prevents further hair loss like hair transplantation. • Follicular Unit Extraction (FUE) – Using brand new technology, a FUE hair transplant is minimally invasive and offers the highest yield of hair regrowth. Extracting individual follicles hair from a donor area and implanting them into your affected area, this treatment boasts a swift procedure, short recovery time, and a minimal amount of scarring.
• Follicular Unit Transplantation (FUT) – Often called the “strip method”, a FUT hair transplant is the “original” hair loss procedure. Tried and tested over decades of use, this surgical procedure takes larger strips of hair from the back of your head, separates the strip into smaller grafts under microscope, and implants these onto your affected area.

The timeline to see the final results of a crown hair transplant can vary from patient to patient. However, in general, it typically takes around 12 to 18 months to see the full and final results of a crown hair transplant.

Here’s a breakdown of the hair transplant timeline:

1. Immediate Post-Transplant Phase (0-2 weeks): This is the healing phase where we ask all patients to follow the post hair transplant aftercare instructions on what to do and how to do it for the first 14 days in order to heal well and have the best chance at great results.
2. Early Growth Phase (2-6 months): During this period (and sometimes prior to 2 months) the transplanted hair follicles may shed, which is a normal part of the process. Patients may experience some redness, swelling, and scabbing in the recipient area during this period. After the initial shedding phase, new hair growth typically begins within 2 to 6 months following the transplant. Initially, the new hairs may appear thin and sparse.
3. Intermediate Growth Phase (6-9 months): By the 6 to 9-month mark, patients usually start to notice more significant growth and increased density in the transplanted area. However, the hair may still be maturing, and the final results are not yet fully evident.
4. Final Growth and Maturation (12-18 months): Between 12 to 18 months post-transplant, the majority of patients will have experienced substantial growth and thickening of the transplanted hair. The hair follicles have matured, and the final results become more apparent.

During this entire timeline, it’s essential to follow the post-operative care instructions provided by your surgeon to optimise the growth and appearance of the transplanted hair. Patience is key, as the process of hair growth and maturation takes time, and individual results may vary based on factors such as genetics and hair quality.
