A female hairline transplant addresses a different anatomical goal compared to a male procedure. Women typically maintain a rounded or oval hairline rather than a squared-off one, and the surgery is designed to respect and recreate that natural shape. The two most common scenarios we treat are a naturally high hairline and early temporal recession at the corners. Grafts are placed to lower the frontal line, soften the temples, or add density to the first few centimetres of scalp. The angle and direction of each implanted follicle is critical: a hairline transplant for women requires precise single-follicular unit placement to keep the result looking completely natural at the frontal edge. Another popular method of hair transplantation is the eyebrow transplant. This is where donor hairs are taken from the back and sides of the head and implanted into the eyebrow areas to appear thicker and fuller.

How Do Hair Transplants for Women Work?

A female hair transplant begins with the same surgical principle as any hair restoration procedure, but the planning stage is fundamentally different.

In most male cases, the goal is hairline reconstruction. In female cases, the goal is density restoration across the top of the scalp, particularly along the mid-parting and crown, following the Ludwig scale pattern of female pattern hair loss. Before surgery, your surgeon maps the recipient area according to your specific Ludwig classification, identifying which zones have the most significant thinning and where density gain will have the greatest visible impact.

Unshaven Extraction in Female Cases

Because most women want to keep their existing hair length, the extraction stage is performed using an unshaven or partially shaven technique. Rather than shaving the entire donor area, small sections are trimmed discreetly beneath the longer surrounding hair. This is one reason female procedures typically extract fewer grafts per session, around 2,000 to 2,500, compared to male cases. It also means the patient can return to normal activities without any visible change to their overall appearance.

Donor Area Stability

Donor area suitability is assessed more carefully in female patients. Unlike androgenic alopecia in men, where the back and sides of the scalp are reliably permanent, female pattern hair loss can affect a broader area of the scalp in some cases. Before surgery, Dr Elgindi or Dr Ditta will examine donor density, hair shaft calibre, and miniaturisation levels to confirm the selected grafts are stable and will not thin after transplantation. Only follicles from areas with no miniaturisation are used.

Implantation and Crown Density Goals

Once the grafts are extracted, they are sorted and implanted into the recipient sites. For mid-parting density, single-follicular units are placed at the parting edge and two-follicular units slightly further back, building a natural graduation of density. For crown coverage, the implantation pattern prioritises even distribution so thinning is concealed across the whole area rather than concentrated in one spot.

Recovery follows the same timeline as other hair transplant procedures. Light scabbing resolves within two weeks, transplanted hair sheds at 6 to 8 weeks, and new growth typically becomes visible from month four. Most female patients see their full result at 12 to 14 months.

The exact price depends on the area being treated, the technique used, and the number of grafts your surgeon estimates you will need. We work on a cost-per-area basis rather than a flat per-graft fee, so the quote you receive at consultation is the price you pay.

Why does a female hair transplant cost differ from a male one? Two reasons:

First, women rarely need the very large graft counts (3,000+) that drive male procedure costs upward, because female hair loss is typically diffuse rather than completely bald.
Second, female cases more often involve unshaven or partially shaven extraction, which adds time to the procedure and limits how many grafts can be transplanted in a single day to around 2,000 – 2,500.

Why Do Women Lose Hair and How Does This Affect Your Chances of Getting a Hair Transplant?

Across the UK, it is estimated that 8 million women are currently suffering with hair loss symptoms. What is perhaps even more alarming is that 40% of women have stated hair loss as a major component in marital problems, and 63% believe it affects their career as well (British Medical Journal, 2005).

The cause of women’s hair loss varies much more than men. Whilst more than 95% of men suffering with hair loss have a condition called male pattern baldness (androgenic alopecia), female hair loss is much more complex.

Around 40% of all female hair loss cases can be explained by hereditary pattern baldness (androgenic alopecia). This leaves 60% of women suffering with hair loss symptoms from extraneous causes. These may include, but are not limited to stress related hair loss, chemotherapy treatments, menopausal symptoms, and harsh styling methods (curling & heat products).

In most cases, a hair transplant can only be offered out to patients who are suffering with a case of permanent hair loss, such as androgenic alopecia. This is where the hair follicles on the head shed and do not grow back.

If you suffer with female pattern baldness or would like a hair transplant in an area where there are no hair follicles (e.g. hairline lowering surgery), then you are likely to be eligible for a hair transplant.

Alternatively, if you suffer with a temporary form of hair loss, such as alopecia areata, then you will likely not be eligible for surgery. This is because your hair is likely to grow back naturally, negating the need for a transplant. In these cases, we may recommend a non surgical alternative such as a PRP hair treatment.

Results and Aftercare

Following your hair transplantation procedure, you will have access to our hair transplant aftercare program and expert guidance, whenever you need it.

It is important to keep the scalp hydrated during the first few days following the procedure in order to protect against infection and minimise scabbing. We provide full instructions for aftercare and are on hand to answer any concerns following your procedure.

It can take 2 to 3 months for the new follicles to take and naturally grow new hair, so regrowth may be uneven at first, but rest assure, you will soon benefit from thicker looking hair. Although every patient’s recovery time and results may differ, most patients see full natural hair growth around 10 months following their procedure.

Yes, hair transplant can be successful in females. Hair transplant procedures have evolved significantly over the years, and they are no longer limited to male patients. Female pattern hair loss is a common condition that can lead to thinning hair and baldness in women. In such cases, hair transplant can be an effective solution to restore hair density and improve the appearance of the scalp.

Whilst there may be a small bit of shedding soon after your procedure, you should not suffer hair loss (in the same area) after your new hair transplant, however it can take around 6 months for the new hair to start to grow and thicken.

During a female hair transplant procedure, local anaesthetic is injected in order to number the area(s). You might feel a slight prick and some discomfort during the initial administration of the local anaesthetic. However, once it takes effect, you should not experience any pain throughout the rest of the hair transplant procedure.

In the days following the hair transplant procedure, it is normal to experience some soreness in the treated areas. However, this discomfort is typically temporary. We also provide you with an aftercare pack that contains antibiotics and pain killers to help alleviate any discomfort and ensure you feel more comfortable during the recovery period. Rest assured that the discomfort is manageable, and you will be well taken care of to ensure a smooth and successful recovery.

The duration of the treatment is determined by the size of the area of hair loss to be treated.

Typically, most female hair transplant procedures last between 6 to 9 hours. However, in larger cases, the procedure might be spread over 2 days to ensure optimal results. Conversely, in smaller cases, the treatment duration will be shorter.

I had an initial consultation with Sara at the start of May and she was so informative, happy to answer any questions and there was no pressure to book whatsoever. The flexibility with dates was incredible and we settled on a date at the end of June. As a woman, my hairline has always been a sensitive topic for me, to the point I’d use a ridiculous amount of hairspray before going out to cover everything in the event of a slight breeze.

I finally started looking for clinics in early 2025 but struggled to find ones in the UK with experience of making a hairline more feminine. Thankfully, an acquaintance recommended Harley Street HTC.

After arranging an initial consultation, they were able to provide me with a loss of four and after photos from other women, How many of our clinics were unable to because, understandably, most of their clientele were men.

I have also quite a nervous patient to say the least. However, Sara could not have been more helpful throughout the whole process. She answered all of my many questions promptly and with compassion and care.

The actual transplant went as smooth as you could hope for, and the clinical team were super kind when I was nervous about them shaving the donor area. For reference I had my temples and my front hairline done in their central London clinic.

During the initial two week recovery periods, they went over and above answering my many questions promptly and calming my anxieties around doing different processes correctly.

Since then, they’ve checked in on me every three months for the past year, where I’ve met with Sara to discuss regrowth and upkeep.

As, unlike most looking to get a hair transplant, I had quite long hair already, I’ll be honest and say that there were some periods where my overall hair did not look great due to the drastically different lengths. However, that could not be helped unless I wanted to shave my whole head and start from scratch.

I am happy to report that after 12 months, my donor area is blending in  perfectly with the rest of my hair. Additionally, my recipient area has seen growth of about 4 or 5 inches and is very dense and full.

Although I didn’t have the widest area done, I’m still over the moon with my results. Harley Street HTC has been fantastic throughout and I couldn’t recommend them enough!
